A single office visit is settled in a fixed order: the charge first eats into any deductible you still owe, then coinsurance splits whatever is left between you and the plan, and a copay sits on top as a flat fee. Working through that sequence explains why the same billed charge costs very different amounts in January and in November.

Coinsurance = (billed - deductible portion) x coinsurance rate; your cost = deductible portion + coinsurance + copay

Because the deductible is untouched, the entire allowed charge lands on you until it is satisfied. Later visits in the same plan year only cost your coinsurance share.
Use the plan's allowed amount if you know it. In-network providers write off the difference between their list price and the allowed amount, so billing the list price overstates your cost.
